Purpose
To promote health by providing members of the Black community with kidney disease with access to health counselling, information and support programs.
Activities in support of the purpose
Activity 1: Quarterly awareness sessions for individuals with kidney disease.
The Foundation will organize and facilitate at least one (1) awareness session per quarter for individuals with kidney disease. These sessions will each be facilitated by a health professional (a doctor or nurse) and each will address a different health related topic relevant to the health and well-being of those with kidney disease. Specifically, the first two topics for such sessions will be high blood pressure and kidney disease and diabetes and kidney disease and will include information about these conditions and how patients can prevent and manage such conditions in the context of their kidney disease. Future topics will be determined based on the interests and needs of the participants.
Activity 2: A bi-monthly peer support group for individuals with kidney disease.
The Foundation will organize and facilitate a bi-monthly peer support group for individuals with kidney disease. These sessions will each be facilitated by a Foundation volunteer and they will be open to individuals who have kidney disease. Each session will have a topic for discussion (which may be supported by a guest speaker, where appropriate) and will also include time for the individuals in attendance to share their experiences in a facilitated environment. Topics will be determined based on the interests and needs of the participants.
